*Nonmember residents who wish to receive the Candidate Member rate must submit a membership application accompanied by a verification letter from their Program Director which should include your resident status and end date for your program.
Spouse/Companion Registration: You may add a spouse/companion registration to your registration for evening social events; this registration type is not eligible for continuing medical education credits. The spouse/companion registration does not include access to any General Sessions, Workshops or Symposia. If you wish to attend the scientific sessions, please register as a nonmember. Two options are available (see the Preliminary Program for more details):

Tripartite Gala: The Tripartite Gala is scheduled for Tuesday, June 13, 2017. Please note:
There is no additional cost for a ticket for full-paying ASCRS Members/Fellows or Tripartite Members for the Tripartite Gala.
The cost for a non-member or additional ticket is $150.
Once onsite, attendees must exchange the Tripartite Gala voucher for a Tripartite Gala ticket in order to attend.

Online and faxed registrations will receive an email confirmation of their registration details. Registrants who submit forms by mail/check will receive a confirmation by post. Be sure to print out and save your email confirmation for your records.

Provision of Your NPI Number
To help exhibitors and sponsors comply with new federal healthcare laws, ASCRS asks its U.S. healthcare provider attendees to supply their 10-digit NPI (National Provider Identifier) number.
As of August 1, 2013, medical device and pharmaceutical companies are required by the Sunshine Act terms of the Patient Protection and Affordable Care Act to use this number to report any payment or other transfer of value (TOV) given to healthcare providers with a minimum value of $10/payment or $100/year to the Department of Health & Human Services (HHS). The Sunshine Act classifies TOV as, but not limited to, gifts, entertainment, certain meals, consulting fees, and honoraria.
If you have not supplied this number, you can add it when you register. This number will be part of the information embedded in your badge’s barcode but will not be printed on the badge.
NPI numbers are public information. If you do not know your NPI number, you can search for your NPI. More information about the NPI is available from the Centers for Medicare and Medicaid Services.
Registrants who wish to cancel their registration should send a request to the ASCRS Meeting Registration Department via email, fax at (847) 427-9656, or mail prior to May 22, 2017. A $75 processing fee will apply to all cancellations. No refunds will be made on cancellations postmarked after May 22, 2017.
